Building an ER MDM Companion with a North Texas Emergency Medicine Partner
We built something for the ER
Emergency physicians do not have time to type full Medical Decision Making (MDM) notes while managing a full board. Most AI scribes assume a calm, conversation-heavy visit. The emergency department does not work that way.
We partnered with a local emergency room organization in North Texas to build an MDM-per-order companion — clinical documentation that assembles as orders are placed, not from a long ambient recording at the end of a shift.
How it works
- Load a case from a library of common ED presentations — or use hands-free voice to drive the workflow without re-tapping the microphone.
- Place orders as you normally would — labs, imaging, medications, consults, and disposition.
- The companion builds MDM documentation in real time tied to the order stream.
- Finalize and copy into the chart workflow — or use a desktop helper for one-click paste into the EHR quick-notes field.
Why it is different
Documentation follows the order stream, not a passive transcript.
| Typical AI scribe | MDM Companion |
|---|---|
| Ambient conversation capture | Order-triggered documentation |
| Best for primary care / UC | Built for interrupt-driven ER workflow |
| Often post-visit summary | Real-time assembly during the encounter |
| Reasoning not always spoken aloud | Captures clinical decisions at the moment orders are placed |
That difference matters in high-acuity environments where physicians think in orders, not narrated paragraphs.
What we shipped
- Case library — bread-and-butter, code/trauma, and specialty presentations
- Typical workup chips — one-tap order bundles per case type
- Hands-free voice — case selection, orders, and finalize without repeated mic taps
- E/M panel — level guidance aligned to MDM complexity
- Chart integration path — copy for quick notes plus optional desktop helper for automated paste
- Shift tools — metrics, shift log, clinical reference sidecar, and in-app guides
